Membership application forms

Does anyone have an electronic copy of the application form they could email me please? I dont have a paper one or a copy of splice at home and am now living in Portsmouth for Uni. Im getting all those lovely emails telling me to pay up but dont have contact details (an theyre not on the site) for membership secs
